failed to connect after deployed on heroku by node.js but local has successed

I can't find problems which are my application could not connect after deployed on heroku by node.js

because my codes and architecture had worked at local server (5000). but when i do deploy, 100% lost.. this is my web page → https://nezca.herokuapp.com

it doesn't work on online below are heroku logs. heroku logs i need your helps.
